If you are applying for the first time to the Access 2 Entertainment Card
If your Access 2 card has expired please fill out the Renewal application form.

If you have lost your card or had it stolen there is no need to reapply. A replacement card cost $20.00 and will maintain its original expiry date. You can pay online for a replacement card or send a cheque or money order made out to:
Please attach a note to the cheque indicating whose card was lost and any other important information such as a change in address. A change in name on the card is considered a replacement card and will also cost $20.
If your card was within 6 months of the expiry date please use the RENEWAL application form.

The Centre for Independent Living in Toronto has generously provided the opportunity for Greater Toronto Area residents to receive the Access 2 Entertainment card for free.
